Mr L Jones is applying for the Renewal of his Hackney Carriage Driver’s Licence for a period of Three Years

The application conforms with all Statutory and Local Authority requirements and all consultations have been undertaken where required. The Licence may, therefore be granted and is submitted for authority to issue under Delegated Powers pending the submission of the outstanding documents listed at the bottom of this form, if the items listed are not presented by the applicant within a reasonable period of time the hackney carriage/private hire drivers licence will be subject to an immediate suspension until such times as the documents required are submitted.

Current Medical Certificate. Please supply as soon as possible and contact the Licensing Office if it is anticipated there will be a delay.

CRB Disclosure Form not submitted. Please arrange for this to be forwarded as soon as possible, together with the necessary supporting documentation.

Photographs not submitted. You will need to supply two coloured passport-sized photographs - one photograph to remain on your file and the other to be endorsed by your GP at the time your medical examination takes place and then attached to your Medical Certificate.
